Designation Letters of Good Standing
Alabama:
If you earned the CIC designation prior to January 1, 2013, fulfilling your required annual update
for your designation also satisfies your state continuing education requirement. To receive the
state credit, include the Letter of Good Standing with your License Renewal form.
Nevada:
Fulfilling your required annual update for your CIC designation provides an exemption for your
state continuing education requirement. To receive this exemption, include the Letter of Good
Standing and the Exemption Form (link to form provided below) with your License Renewal form.

New Jersey:
Fulfilling your update requirement for your CIC or CISR designation also satisfies your state
continuing education requirement. Fax your CE1A Certificate of Completion and Alternative
Continuing Education Credit Application (link to form provided below), to the New Jersey
Department of Insurance, not more than 90 days prior to your renewal.

Puerto Rico:
Fulfilling your update requirement for your CIC or CISR designation also satisfies your state
continuing education requirement. To receive credits send an email with a copy of your letter
and license information to Miguel A. Ramos Torres at mramos@ocs.pr.gov
Utah:
Fulfilling your required annual update for your CIC or CRM designation also satisfies
your state continuing education requirement.
To renew your license: Save your Letters of Good Standing to your desktop and title as “CIC Certificate.” Logon to http://www.sircon.com/utah and pay your renewal fee, after paying the fee click on the box at the top that says “attach supporting documentation” and attach your Letters of Good Standing. It may take several minutes for your documents to load; print confirmation ID information. Send email confirmation of completion of the process to Michael Covington at mcovington@utah.gov and include your license number. If you paid your renewal fee and did not upload your Letters of Good Standing, contact Michael Covington for instructions to complete the renewal process.

ALTERNATIVE CONTINUING EDUCATION CREDIT APPLICATION APPROVED INSURANCE DESIGNATION FORM |
Purpose: Verification form for Individual licensee requesting continuing education credits for meeting the continuing education requirements of an approved insurance designation.
Please submit this form to the governing body of the appropriate insurance designation for completion. The completed form may be faxed by the licensee to the Office of Consumer Protection Services - Producer Education at (609) 984-5263 or Mail to: Office of Consumer Protection Services, Department of Banking & Insurance, PO Box 329, Trenton, NJ 08625-0529

Designation Grantor Statement: To be completed by representative of the organization granting the approved insurance designation. This form should be returned to the Producer requesting consideration for alternative continuing education credit evaluation.
I hereby certify that the above designee has completed the continuing education requirement to maintain the approved insurance designation identified above for the Jan - Dec period and remains a member in good standing. The Department will award 12 continuing education credits for each full year of the license term that the Designation is maintained in good standing.
